About Chronically Informed

According to the Center for Disease Control, “Chronic diseases are defined broadly as conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both.”

 

More than 50 percent of the people in the United Stated live with a chronic illness, and more than 27 percent have multiple chronic conditions.*
