Hello, Chronically Informed Community! Whether you’ve been with us all along or you’re new to the platform, we’re so glad you’re here! We’re so grateful to each and every one of you who has checked out the platform, made a video, followed us on social media, liked and shared our posts, and followed our progress. The high-fidelity prototype of Chronically Informed is currently being finalized so that it can be piloted by patients with chronic illness and the professionals who support and advocate for them. We won’t be posting videos while the site is under construction, but we still need your help! Please let us know if you are a patient living with chronic illness who is interested in sharing your story of success and/or practical technique for navigating the medical system. Is making a video not your thing? Are you a person who doesn’t live with chronic illness? Volunteers Needed to Make Videos

Chronically Informed is looking for people living with chronic illness to volunteer to make videos for the platform! This is an amazing opportunity to advocate for other patients with chronic illness and share stories of success and practical techniques for navigating the medical system. We are grateful to be partnering with the Chronic Care Collaborative in this work, and we are incredibly excited for this project to grow. In order for this to happen, we need YOU! We are looking for videos on a variety of topics, including but not limited to Working with Providers, Using the Pharmacy, and Health Insurance Tips.
